Tri-Med Home Care Services has earned The Joint Commission’s Gold Seal of Approval® for Home Health Care Accreditation by demonstrating continuous compliance with its performance standards. The Gold Seal is a symbol of quality that reflects a health care organization’s commitment to providing safe and quality patient care.
Tri-Med Home Care Services underwent a rigorous, unannounced onsite review on November 9, 2021. During the visit, a team of Joint Commission reviewers evaluated compliance with Home Health Care standards spanning several areas including performance improvement, infection prevention and control, leadership, medication management, rights and responsibilities of the patient, environment of care and management of information systems.
The Joint Commission’s standards are developed in consultation with health care experts and providers, measurement experts and patients. They are informed by scientific literature and expert consensus to help health care organizations measure, assess and improve performance. The surveyors also conducted onsite observations and interviews.
“As a private accreditor, The Joint Commission surveys health care organizations to protect the public by identifying deficiencies in care and working with those organizations to correct them as quickly and sustainably as possible,” says Mark Pelletier, RN, MS, chief operating officer, Accreditation and Certification Operations, and chief nursing executive, The Joint Commission . We commend Tri-Med Home Care Services for its continuous quality improvement efforts in patient safety and quality of care.”
“I am extremely proud of this achievement” says Walter Kaltenbach, Chief Operating Officer. “Our staff is very committed to improving the health and safety of our patients. The Joint Commission standards of care offer great value in evaluating our internal policies/procedures, staff competencies, organizational communication, quality standards and assuring maximum performance”.
Tri-Med’s leadership spent several months preparing for the survey. Staff meetings were held to itemize and review each joint commission quality standard. Revisions were made to our internal policies in effort to meet these standards as well as improve key areas that affect quality and safety of patient care. A companywide “Performance Improvement Plan” was established to assess the provision of cost efficient, high quality of services on a continuous basis. Adhering to these quality standards requires continuous monitoring through the collection of data related to key processes and services. All levels of staff, patients and key industry stakeholders participate on Tri-Med’s Performance Improvement committee to evaluate and ensure that Tri-Med’s commitment and services meet the joint commission standards of care. We are therefore very pleased to share this endeavor with our patients, families, referral source partners and the communities for whom we service.
